physicians at Issaquah OBGYN and Gynecology, PLLC, participate in most insurance programs. The best way to find out if we participate
in your particular plan is to contact your insurance company directly to inquire. You may wish to check your benefits as well.
Each insurance plan has its own requirements and restrictions. Being fully informed and aware will help you in requesting
services and avoiding unnecessary expense for which you would be responsible.
We request the chance to copy your insurance card at the time of your office
visit. We are happy to bill your insurance for your care and will also provide your insurance information to the lab should
any tests be necessary. Co-pays must be paid before the service is provided. Failure to pay your co-pay at the time of your
visit may subject you to an additional fee.
If you have no insurance coverage for your care with us, we require that
your visit be paid at the time you are seen. Pre-payment may be expected for certain elective services. We accept personal
checks, cash, Visa and MasterCard for your convenience.

Maternity Care We understand that this is a special time for you and your family. Maternity care is a "global" service that encompasses
your prenatal office visits, the delivery of your baby and your postpartum care. The global fee is billed at the time of your
delivery. Additional services, including ultrasound scan, lab tests and other required testing is billed separately by whoever
provides the service. Our practice is committed to providing you with the best healthcare possible. We

circumstances hinder your ability to pay your account, we encourage you to contact us for assistance in managing your account.
